• Applicants name to be displayed on the document

• Applicant name shown on document must match the applicant name that is registered in Onsite Track Easy (However shortened first names can be accepted such as ‘Chris’ for ‘Christopher’)

• Certificate must indicate completion of Health & Safety Representative Training

• Certificate must be issued by an approved provider by Worksafe Victoria as per link: http://www1.worksafe.vic.gov.au/vwa/serviceproviderdirec.nsf/list?openForm (select OHS Training providers)

• Issue Date must be recorded in Onsite Track Easy as the date of completion that is shown on the document

• No expiry date is required
