- Date of Birth (DOB) for applicant must be shown on the Fitness to Drive Report and must match Date of Birth which is registered for the applicant in Onsite.
**If the Date of Birth (DOB) is not shown on the Fitness to Drive Report then the secondary check is the person's drivers licence number instead of the date of birth.
VERIFICATION OF RESULTS: - Fitness to Drive Report page to be present within uploaded file
- Assessment outcome section must be completed
NOTE: If a medical indicates the applicant 'Unconditionally meets the medical criteria for fitness to drive' or 'Conditionally meets the medical criteria for fitness to drive' Pegasus staff will accept the Fitness to Drive Report NOTE: If a medical indicates the applicant ‘Temporarily does not meet the medical criteria for fitness to drive' or 'Permanently does not meet the medical criteria for fitness to drive' Pegasus staff will reject the Fitness to Drive Report
VERIFICATION DETAILS: - Health Professional's details must be completed (First and Last name OR First name initial with Last name and signature should be present)
- Date of Assessment must be recorded
- Black and white or colour is accepted
| - The issue date in Onsite should be recorded as the date the assessment or appointment took place.
- The expiry date in Onsite should be calculated based on the individuals age:
- If aged 49 or under - 3 yearly
- Age 50 or over - annually
- Then this calculation needs to be checked against what the doctor has ticked in the assessment form as the review date. If there is a difference in the calculated date and the review date that's been ticked we need to query this.
