Personnel Income Protection

• Company Name needs to match. If it doesn’t match, evidence must be provided to link the business names together. Ideally, this is a letter or download from the ABR showing the linkage.

• There is no minimum amount for Income Protection and the amount does not need to be recorded

• Must be current

• Must specify the state in which the certification has been uploaded for

• Must specify Income Protection or Accident and Sickness

• Issue Date = Start of coverage date listed on policy

• Expiry Date = Coverage end date listed on policy (If the policy is paid on a monthly 
  basis the expiry will be set when the next payment is due)

• Insurer = Insurer will be listed in policy content

• Policy Number = as specified on the policy

• Item = Income Protection or Accident and Sickness as specified on the policy

• Amount Description = not required

• Amount = not required

Motor Vehicle Insurance

Company Name needs to match. If it doesn’t match, evidence must be provided to link the business names together. Ideally, this is a letter or download from the ABR showing the linkage.
- 3rd party personal and property or Comprehensive cover to be listed
- No minimum coverage requirement
- Duration as per document expiry date
- Black & White or colour copy acceptable
- ABN to be verified
- Must be current

• Motor Composite Insurance

• Industrial Special Plant Insurance (Motor Vehicle’s need to be listed)

• Commercial Motor Insurance 

• Commercial Motor Vehicle and Machinery Insurance

• Comprehensive Insurance

• Amount = as per policy (if shown on policy) UNLESS the policy is for comprehensive, in which case the amount can be 0

Plant & Equipment Insurance

Company Name needs to match. If it doesn’t match, evidence must be provided to link the business names together. Ideally, this is a letter or download from the ABR showing the linkage.
- No minimum coverage requirement
- Duration as per document expiry date
- Black & White copy acceptable
- If there are any special conditions these must also be uploaded

• Mobile Plant & Machinery

• Motor Vehicle Fleet Insurance (must have Equipment listed and not just Motor Vehicle)

• Industrial special Plant Insurance

• Driveline Plant and Machinery Policy

• General Property Insurance

• Commercial Motor Vehicle and Machinery Insurance

• Yellow Cover (Must include list of equipment)

• Issue and expiry date to be recorded

• If policy is paid monthly please enter expiry as paid to date

Public and/or Product Liability Insurance

• Company Name needs to match. If it doesn’t match, evidence must be provided to link the business names together. Ideally, this is a letter or download from the ABR showing the linkage.
  - Minimum amount of $20 million dollars AU for any one occurrence
  - International policies accepted provided it is equivalent to $20 million dollars AU
  - Must be in AUD
  - Duration as per document expiry date
  - Black & White copy acceptable
  - Must be current

  - General Liability
  - Business Liability
  - Legal Liability
  - Umbrella Liability

Professional Indemnity

  - Details of insured activities and amount insured for each
  - Minimum $1 million AU Insured amount
  - Must be in AUD
  - Duration as per document expiry date
  - Black & White copy acceptable
  - Must be current

  - Civil Liability
