Subject/Topic of the Training
Instructor Name (for Safety Meeting/Toolbox Talk) OR Training Company Name (for certificate / refresher)
Employee First Name and Last Name
Employee Signature (Only required for Safety Meeting/Toolbox Talk)
Date
Training documentation can be in the form of a:
Certificate - Either issued internally by the employer company or by third party training organization
Safety Meeting/Toolbox Talk - Issued internally by the employer company
Refresher Training - Either issued internally by the employer company or by third party training organization
Issue Date = Date of Completion
Expiry Date= 12 months from date of completion

C10 General Electrician License
Electrician’s License can have either the Electrician’s First and Last Name or the name of the Corporation/Business
Electrician’s License Header in Upper Right Corner should state “DCA (Department of Consumer Affairs” Middle of Card should state “ Contractor’s State License Board Active License” or “Certified General Electrician” and Footer may say State of California DIR (Department of Industrial Relations) and right upper corner should display the California Circular Seal (The Great Seal of the State of California”
Electrician’s License should have a six-digit License Number listed as either License Number xxxxxx or No. xxxxxx
Electrician’s License should state the Entity as either CORP or Individual
Electrician’s License should have License Holder Name or Business Name
Electrician’s License should have a Classification(s0 as C10
Electrician’s License should have an expiration date; It may show a Valid Date as xx/xx/xxxx to xx/xx/xxxx or Expiration Date xx/xx/xxxx
Electrician’s License may or may not display a photo of the electrician on it
Electrician’s License may have the California State License Board email listed on bottom of card as http://www.cslb.ca.gov along with the CSLB Logo
Issue Date = Only enter this If there is a validity start date shown on licence
