Government Issued Identification

• Upload a copy of the identification that lists the following:

• Name on provided identification to match the name registered in the system

• Photo to match person

• Date of Birth (DOB) to match applicants DOB in Onsite

• Must be current

• Must be issued by a US government authority

• Must be in color

• Scanned document to be clearly legible

• Accepted forms of government identification: Driver’s License/ driver authorization card, State Issued ID card, permanent resident card and passport

Note: Temporary / interim evidence such as a receipt can be accepted for temporary amount of time only while awaiting the arrival of the formal evidence. If there is no interim acceptance time frame shown on the document, accept this for 1 month only.

• Issue Date: Only list this if there is one displayed on the document

• Expiry Date: list expiration date as it is shown on the document
