Operator Forklift.Qualified.Lift Director (Assembly/Dis-assembly Director)

  • Acceptable evidence can be in the form of:

    • Certificate

    • Refresher Training

  • Evidence must state completion of Lift Director (Assembly/Disassembly Director) or Lift Planner

  • Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

  • Colour copy or black and white copy is acceptable 

  • Issue Date = Date of completion

  • Expiry Date = 1 year

Operator Crane.Certificate.Crane Operator Certification

  • Acceptable evidence can be in the form of:

    • Certificate

    • Refresher Training

  • Evidence must state completion of Crane Operator

  • Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

  • Colour copy or black and white copy is acceptable 

Issue Date: Date of completion

Expiry Date:

  • If there is an expiry date shown on the evidence that demonstrates 3 years or less validity from the date of completion - Record the expiry date as it is shown on the evidence.

Note: If the expiry date on the evidence shows it is already expired, this will need to be rejected and a current form of training evidence will need to be supplied

  • If there is an expiry date that is shown on the evidence that gives more than 3 years validity OR if there is no expiry shown on the evidence - The expiry date can be recorded in the system as 3 years from the date of training completion

Operator Excavator.Awareness.Trenching & Excavation User Training Awareness

    • Refresher Training

    • Safety Meeting/Toolbox Talk

  • Evidence must state completion of Trenching & Excavation User Training Awareness

  • Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)

  • Colour copy or black and white copy is acceptable 

  • Issue Date = Date of completion

  • Expiry Date = 1 year

  • Samples of acceptable course titles include (but are not limited to)

    • Scaffold Building Competent Person

    • Scaffold Competent Person

    • Scaffolding Safety for the Competent Person

    • Competent Person - Suspended Scaffold

  • Applicant's name should be displayed on the evidence and should match the name registered in the system (however shortened versions of names such as 'Chris' for 'Christopher' is acceptable)
