• Thiess issued Order 43 Medical Form must be uploaded
• First name, Last name and date of birth on the form must match the applicants details in the system

BHP COAL PTY LTD workers only: Can upload the medical results pages/traffic light pages instead of obtaining a Thiess Order 43 Medical Notification letter.

All future Medicals without the Traffic Light page can be approved for BHP workers only

• Issue Date = Not required
• Expiry Date = The date that will be expiring soonest out of the Next Order 43 Assessment or Date of next chest X-ray to be recorded

• Name and DOB on medical to match the person registered
• Drug and alcohol test completed by an NATA accredited provider
• MUST have been completed within 30 days of the role application
• 0 BAC and All Negative for Drugs.

• Issue date to be recorded
• D & A screening must be in accordance with AS4308 Urine testing shall screen for:
• - Amphetamines
• - Benzodiazepines
• - Opiate
• - Cannabinoids metabolites
• - Cocaine Metabolites
