- Position must be completed
- Contractor Company must be completed
- Must select Primary WHC site
- Induction(s) required – must be completed
- Approval section: must at minimum be signed by a contract owner / coordinator
- In addition, tradesperson roles must be signed off by a person on the authorised approvers list relevant to the role e.g. Electrical Tradesperson signed by Manager Electrical Electrical Engineering.

The Manager of Mining Engineering (MME) or CHPP Manager need to sign for the role 'All Open Cut - Operator'. Page 3 Roles: a role in the Open Cut section must be selected Page 4, 5 & 6
