  • This is a free text field but ideally the medical expiry date will be entered in the following format: DD/MM/YYYY
  • This date must align to the date shown on actual medical documentation and must be current but expiry date cannot be more than 2 years in the future. Applications will be returned if a past date is entered.
  • Copies of actual medical documentation should not be uploaded / stored in this system

When processing the submitted application, the verification team will use the input expiry date information to assign a site-based competency titled: Medical.Certificate.QAL/RTAY/BSL (3 Site Medical) Compliant Medical to the workers profile.

Rio Tinto site contacts may ask to see copies of supporting documentation on site.

  • Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)
  • Document must be a Statement of Attainment
  • Statement of Attainment must show HLTPAT005 Collect Specimens for Drug of Abuse Tresting (or equivalent)
  • Must be Issued by an RTO with Nationally Recognised Training logo
  • Black and white or colour is acceptable
  • Scanned document to be clearly legible
  • Issue Date: Course completion date as shown on document
  • Expiry Date: Not Required

Operator.Qualified.RTAY Respirator - Disposable P2; 30923871
  • First and last name must match applicant's name in Onsite

Shortened versions of first names are accepted, such as:
"Chris" instead of "Christopher"
"Wil" instead of "William"

Shortened versions of last names are accepted, such as:
"Herrera" instead of "Herrera Trujillo"
"Mohd" instead of "Raza Hasan Mohd"

  • The following evidence can be accepted:

    • RTAY Recognition of Prior Competency

    • Rio Tinto Record of Completion

    • Theory Assessment or Site Authorisation
      All assessment pages within upload
      Matching competency name and code (if with checkbox, this should be completed)
      Applicant's signature and signed date
      Assessor's complete name, signature, and date
      Must show that the person is competent and authorised to operate

NOTE:

  • If Site Authorisation, the RCC Assessment or CTO Assessment section must be completed

  • If Theory Assessment, when RCC is completed, the authorised leader is required to verify with a signature that the applicant is authorised to operate

  • The learning and development section of the form does not need to be completed

Issue date = If a certificate has been provided, use the course completion date as issue date. If Theory Assessment or Site Authorisation, use the assessor's signature date.

Expiry date = 2 years from issue date

Medical Certificate- Medical/ General Practitioner 
  • First and last name must match applicant's name in Onsite

Shortened versions of first names are accepted, such as:
"Chris" instead of "Christopher"
"Wil" instead of "William"

Shortened versions of last names are accepted, such as:
"Herrera" instead of "Herrera Trujillo"
"Mohd" instead of "Raza Hasan Mohd"

International Medical Practitioners – need to provide their registration from the Medical Board of Australia which will advise the type of the registration ie Limited; Provisional or General Registration.  This registration will advise that they have had their overseas qualifications assessed and have completed the formal process assessment program required to work in Australia.  International medical qualification/s will still also need to be provided.

Medical.Competency.BSL Medical - Blue Card
  • Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)
  • A blue card must be uploaded (no other medical records or results are to be uploaded)
  • The Date of birth on the card to match the registered DOB on the system
  • Front and back is required only if all required information is not shown on the front of the card
  • Must be clear and legible
  • Must be current


NOTE: old site access cards that have medical expiry date referenced but do not explicitly show they are issued by an actual medical provider are not acceptable

Medical.Competency.BSL Medical - Yellow Card (Respirator wearing areas only)
  • Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)
  • A yellow card must be uploaded (no other medical records or results are to be uploaded)
  • The Date of birth on the card to match the registered DOB on the system
  • Front and back is required only if all the required information is not shown on the front of the card
  • Must be clear and legible
  • Must be current


NOTE: old site access cards that have medical expiry date referenced but do not explicitly show they are issued by an actual medical provider are not acceptable

  • Issue date = The date of the most recently completed 'Spirometry' Component – this will be either the Pre/Post Spirometry date or the Updated Spirometry date.

If the card provided is branded “JOBFIT”, the issue date to reference is the Yellow Card LFT

  • Expiry date = 6 months from issue date UNLESS earlier spirometry review is mentioned in the conditions
