• Can be either the Statement of Attainment Certificate OR the STD 11 Card (Green Card or 'issued on achievement of competence in:')
• All 6 of the following units must be listed:
• RIICOM201, RIIGOV201, RIIOHS201 (now replaced by RIIWHS201), RIIRIS201, RIIERR205 and RIIERR302
• Black and white or colour acceptable

Note: If certificate Standard 11 - Surface Operations - PARTIAL COMPLETION – please enter an expiration date of 6 Months

• Issue Date: as per the issue date identified on the Statement of Attainment or the ID card

  Expiry Date:

  • Standard 11 - Surface Operations=   five (5) years after the issue date identified.
  • Surface Operations - PARTIAL COMPLETION= six (6) months after the issue date identified
