  • All 6 of the following units must be listed:
  • RIICOM201, RIIGOV201, RIIOHS201 (now replaced by RIIWHS201), RIIRIS201, RIIERR205 and RIIERR302
  • Black and white or colour acceptable

Note: If certificate Standard 11 - Surface Operations – “Statement of Completion” – please enter an expiration date of 6 Months

Issue Date: as per the issue date identified on the Statement of Attainment or the ID card
