• Document must be a Statement of Attainment
• Statement of Attainment must show RIIWHS204 Working Safely at Heights
• Must be Issued by an RTO with Nationally Recognised Training logo.  
• Black and white or colour is acceptable
• Scanned document to be clearly legible

• Expiry date to be entered as 2 years from the issue date
