Equipment.Test.HPD Fit Testing

• Name on test results to match the person registered (However shortened first names such as 'Chris' for 'Christopher' are accepted)

• Overall FF must state Pass = Yes

• Signature is not required

• Testing completed as part of a medical can also be accepted – the Summary of the medical must show a green tick, a green score, a green pass or something equivalent

• Issue to be entered as Test date on form

• Expiry Date to be entered as 2 years from issue date
