Category 1 to be displayed on the fit for duty page
Black and white or colour is accepted
Drug and alcohol testing results are not required to be displayed within the uploaded file. However, if the results are included, they must indicate a negative test result
A 'Subject to Review' medical cannot be extended past the natural expiry date set by the system for the original medical.
The issue date should be recorded as the date the assessment took place. If a medical document has more than one assessment date use the assessment date from the fitness for duty page.
Calculating the Expiry Date Medical Assessments for Category 1 workers are to be carried out prior to commencing work and then at intervals outlined below (Unless the worker's medical states otherwise):
Expiry Date Up to 46 years old - 5 yearly Ages 47, 48 and 49 (the expiry would be set as their 52nd birthday) Age 50-59 - every 2 years From age 60 - renewed annually
NOTE:RIW Medical screenshots will be accepted with same rules above apply. If this is submitted, the AHP name and signature are not required

Name on medical must match applicant name (shortened version of name is accepted eg. Chris for Christopher)
Date of Birth (DOB) for applicant must be shown on the medical.
Verification of Results
Individual’s name must be on fitness for duty page.
AHP name and signature must be present on fitness for duty page.
Fit for duty page must be included.
Fit for duty section must be ticked.
The type of medical must be written on the form (periodic, pre-placement or change of category)
Note: A medical stating limitations or clarifications is not to be uploaded or accepted against this competency.

The expiration date for this competency should be calculated to occur on the applicants 40th birthday. If the applicant had a full health assessment completed within 5 years of their 40th birthday the expiration date should be set to 5 years from the date the medical was completed.
After the age of 40, medicals are to be completed every 5 years
NSW ONLY: At any age, the expiry date is 5 years from the date the medical is completed.
