• Employee name shown on Form must match the applicants registered name in system

• Position on the form needs to match the competency selected

 Acceptable Positions:

• Electrical Superintendent

• Tunnel Superintendent

• Electrical Leading Hand

• Electrical Supervisor

• Electrical Tunnel Worker

• Tunnel Worker Leading Hand

• Tunnel Shift Boss

• Tunnel Worker

• All boxes need to be ticked in acknowledgement

• The worker and supervisor's name, role, signature and date must be reflected on the form

Site - Position Description

• Position Title on the form to match Role selected in portal

• Employee Name, Signature & Date required

Expiry date: Not Required

• Name on Appointment Letter to match

• Must be on a Company or JV Letter head

• Document must be signed by employee and Project Manager 

• Must be Magazine Keeper Appointment
