AS/NZ 4801 Certificate	Business Name on Certification to match the organisation registered. If it doesn't match, government certified evidence must be provided to link the ABN or business names together. This is ideally a letter or download from ABR showing the linkage. Black and white or colour is acceptable Must be certificated AS/NZS 4801 JAS-ANZ stamp to be present on 4801- If a certificate is supplied that is a combined 4801 and 18001 there will be NO JAS-ANZ stamp present. This is acceptable. JAS-ANZ international accreditation equivalents, for example IAS, AIAS, UKAS, UAF, UAS etc are also acceptable Upload all pages of the certificate Previously, OHSAS 18001 could be accepted in lieu of AS/NZ 4801, however: From March 12, 2018: ISO 45001 will start to be issued in lieu of OHSAS 18001. From September 30, 2021: OHSAS 18001 will no longer be accepted - ISO 45001 MUST be supplied From Jul 13, 2023: AS/NZS 4801 will no longer be accepted as certified organisations are required to migrate to ISO 45001:2018.
