• Applicants name to be displayed on uploaded evidence and to match applicants registered name

• Date of birth on document must match applicants date of birth in the system

• Copy must be clear and legible

• The birth certificate must be issued by Australian State Government appointed agency

• Black and white or coloured copy of the document is accepted

• No Expiry date is required to be recorded in the system

Identity.Govt Licence/Authority.Australian Passport

• Applicants name to be displayed on uploaded evidence and to match applicants registered name

• Date of birth on document must match applicants date of birth in the system

• Must be valid passport

• Must be issued by relevant Government Authority

• Black and white or coloured copy of the document is accepted

• Photo on the ID must match the applicants photo on the data application

• No Expiry date is required to be recorded in the system

Identity.Govt Licence/Authority.Australian Proof of Identity Card

• Applicants name to be displayed on uploaded evidence and to match applicants registered name

• Valid Proof of Age card OR Photo Card to be uploaded

• Must be issued by relevant Australian state Government Authority

• Black and white or coloured copy of the document is accepted

• No Expiry date is required to be recorded in the system
