Verification of Results
There must be an Overall ‘Impression’, ‘Assessment’ or ‘Determination’ page included within the uploaded file which indicates the applicant is: Fit for duty / Suitable to carry our duties of proposed employment without restrictions
The medical must be completed within the last 2 years
NOTE: Medical recommendations which may be included in this result are not necessarily restrictions
If there are restrictions on this page: Avetta staff will escalate for client review unless it is only restricted for one of the following reasons where escalation is not required:
Requires prescription safety glasses
Hearing loss / hearing aids
Drug and alcohol results must be displayed - Minimum of ‘0’ or ‘negative’ recorded for alcohol testing result and ‘negative’ for drug result
The Chest X-ray report (ILO report), Medical Report and Drug and Alcohol screen, can be uploaded as a single file. All three are needed to comply with the medical requirements for site access
Acceptable Chest X-ray results indicate clear lungs and normal findings (e.g., heart not enlarged/normal) or only mild conditions
Restricted medicals will be escalated by Avetta for Client Review where the restriction[s] relate to anything other than:
Must be completed by Medical practitioner / provider
Issue Date = Date of examination / date medical assessment was conducted. If ResHealth Medical has been submitted, please use the assessment date as issue date.
Calculating the Expiry Date
The medical must be completed within the last 2 years
If the medical file specifies a review / expiry date which has been nominated by the medical practitioner, this will be recorded as the expiry date in the system
If the medical file does not specify a review / expiry date, Pegasus staff will calculate the expiry as follows:
Enter the expiry as the date which is set to occur 3 years from issue date
Medical.-.CRO Health Surveillance Report (RCS)

Types of Acceptable Respiratory Health Surveillance Reports:
Resources Safety and Health Queensland (ResHealth) – Health Surveillance Report for Respiratory health surveillance of mineral mine and quarry workers
Coal Mine Workers’ Health Scheme (CMWHS) – Section 4 provided by Resources Safety & Health (ResHealth)
The Health Surveillance Reports must not be older then 5 years from the date of completion

The expiration of the competency is identified at Section 2(i) UNLESS the following occurs:
If section 2(j) states that an additional respiratory examination is required prior to the date of the next full respiratory health surveillance (Section 2(i)) then the date of the examination stated in Section 2(j) becomes the expiry date.
