• If a Sole Trader the name on Licence to match the person registered

• If a Company, the name on Licence to match the company registered

• Building to be listed

• Must be issued by relevant government department based on state of issue

• Duration as per licence

• Black and white of colour is acceptable

• Relevant government department based on state of issue must be present

Note:  Otis will accept a screen shot showing that the licence is current for an interim of 4 weeks and then the licence needs to be supplied
