1	AS/NZS-4801 Certification	Business name on certification to match the organisation registered. If it doesn't match, evidence must be provided to link the NZBN or business names together. This is ideally a letter or download from NZBN office showing the linkage. Note: If a letter of linkage is supplied, the documentation will need to be sent to site for their approval prior to verification Must be certificated AS/NZ 4801 certificate JAS-ANZ stamp to be present Black and white or colour is acceptable	Issue Date = As per certification Expiry Date = As per certification
2	ISO-14001 Certification	Business name on certification to match the organisation registered. If it doesn't match, evidence must be provided to link the NZBN or business names together. This is ideally a letter or download from NZBN office showing the linkage. Note: If a letter of linkage is supplied, the documentation will need to be sent to site for their approval prior to verification Must be certificated ISO 14001 certificate ISO 14001 stamp to be present Black and white or colour is acceptable	Issue Date = As per certification Expiry Date = As per certification
3	ISO-18001 Certification	Business name on certification to match the organisation registered. If it doesn't match, evidence must be provided to link the NZBN or business names together. This is ideally a letter or download from NZBN office showing the linkage. Note: If a letter of linkage is supplied, the documentation will need to be sent to site for their approval prior to verification Must be certificated ISO/OHSAS 18001 certificate ISO 18001 or OHSAS 18001 stamp to be present Black and white or colour is acceptable From March 12, 2018: ISO 45001 will start to be issued in lieu of OHSAS 18001 From March 12, 2021: OHSAS 18001 will no longer be accepted, ISO 45001 MUST be supplied	Issue Date = As per certification Expiry Date = As per certification

7	SafePlus Onsite Assessment	Business name on certification to match the organisation registered. If it doesn't match, evidence must be provided to link the NZBN or business names together. This is ideally a letter or download from NZBN office showing the linkage. Note: If a letter of linkage is supplied, the documentation will need to be sent to site for their approval prior to verification Must be certificated Safe-Plus certificate Black and white or colour is acceptable	Issue Date = As per certification Expiry Date = As per certification
