  • Must submit evidence showing completion of Industry Safety Induction
  • Forms of evidence that can be accepted:
    • Certificate of completion issued by a Registered Training Organisation (RTO); or
    • Experienced Worker Declaration. Must be fully completed

Issue date to be recorded

If Exp Worker Dec, authorisation date to be recorded as issue date
