- The 'Pegasus Order 43 Medical Notification' form showing the Thiess logo must be supplied
NOTE: If the document uploaded includes a copy of the actual medical with the notification form, Pegasus staff will return this. Only the notification form can be supplied and accepted - First name, Last name and date of birth on the form must match the applicants details in the system

- All future Medicals without the Traffic Light page can be approved for BHP workers only

- Expiry Date = The date that will be expiring soonest out of the Next Order 43 Assessment or Date of next chest X-ray to be recorded

| Medical.Assessment.Drug and Alcohol Assessment | - Name and DOB on medical to match the person registered
- Drug and alcohol test completed by an NATA accredited provider
- MUST have been completed within 30 days of the role application
- 0 BAC and All Negative for Drugs.

- D & A screening must be in accordance with AS4308 Urine testing shall screen for:
- - Amphetamines
- - Benzodiazepines
- - Opiate
- - Cannabinoids metabolites
- - Cocaine Metabolites
