Must be an Australian Government issued document (birth certificate, 18 plus card)
Name and date of birth on the evidence to match the person registered
Must be legible

Note: Receipt of purchase for driver's licence is accepted for Middlemount Coal (Expiry Date = 60 days from purchase).
