• Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)
• Must be 'CTO' training. If 'CTO' is missing from the certification name it is a different training
• Both training numbers (30772792 and 30404390) must be listed on the document. If there are checkboxes next to them, the box must be checked
• All assessment pages must be attached and completed
• "RTAY Confined Space Aware and Standby CTO" is an acceptable replacement for this training
• Acceptable replacement for this training must include the following on Recognition of Prior Competency document:
  • RTAY C5 Confined Space Awareness (30404390)
  • RTAY Confined Space Entry - CTO (30772792)
• Assessor signature and date must be completed
• Black and white or color is acceptable
• Scanned document to be clearly legible

• Issue Date to be entered as date of training completion
• Expiry date to be entered as 2 years from the issue date

Confined Space.Statement of Attainment.Enter Confined Space MSMPER205

• Document must be a Statement of Attainment
• Statement of Attainment must show Enter Confined Spaces (MSMPER205)
• Must be Issued by an RTO with Nationally Recognised Training logo
• Black and white or colour is acceptable
• Scanned document to be clearly legible

• Issue Date to be entered as date of training completion
• Expiry date to be entered as 3 years from the issue date

• Statement of Attainment must show Enter & Work in Confined Spaces (RIIOHS202A or RIIWHS202E)

• Expiry date to be entered as 3 years from the issue date
