Letter of Competency must be a completed version of the Letter of Competency downloadable template and be on their Company Letterhead

or,

Letter of Competency must:-

- be on a Company Letterhead (including company address & contact info)

- include Issue Date recorded (within 1 month)

- include Company ABN

- be signed by senior manager of company

Letter of Competency must include:-

1. Company Trading Name

2. The name of applicant

3. The occupation of applicant

4. A list of qualifications and tasks to be completed during time onsite

5. Period of employment with the current company

6. Experience in years/months in trade

7. Experience in years/months in the mining industry

8. Details of experience in tasks to be performed onsite

9. Details of training that has been provided by the employer with regard to work procedures, risk management, hazard management, the company's WHS safety management system and employee obligations

10. A list of equipment where maintenance experience exists for applicant

11. Applicant performance relating to accidents or incidents

12. Date/s of current NSW Order 41/41 medical & Chest X-Ray

13. Level of Supervision required for applicant

14. Will Hot Work be performed by applicant?  Yes/No If yes, supply MDG25 Awareness Training qualification

An Apprentice role must detail #13 Level of Supervision required for applicant

A Trainee role must detail #13 Level of Supervision required for applicant plus include a list of current Training Modules

• Issue Date = date on the letter (must be dated within 1 month at the time of submission
• Expiry Date = no expiry date to be recorded
