• The form should be fully completed to be accepted

• All pages of the form must be included within the uploaded file
• The role of the person is listed i.e. Operator / Coal loader.
• The primary location must be listed.
• The 'induction required' would ideally be listed on the form however if this has been missed in completing the form it can still be accepted
• For sign off:
  The contract owner or Contract coordinator always needs to sign.
  The mechanical or electrical is only required if the role is linked to any mechanical or electrical role.
  The Manager of Mining engineering needs to sign for ALL underground trades and Operator for Narrabri. 
• NOTE: MME sign off not needed for surface role at Narrabri

• The Manager of Mining engineering (MME) needs to sign onto the form for anyone that selected the role titled 'All Open Cut - Operator'.
  NOTE: Gunnedah CHPP does not have a MME – therefore this signature is not required for applicants that have indicated Gunnedah CHPP is their primary WHC site even if the 'All Open Cut - Operator' role has been chosen.
  

Only version 8 ,9 and 10 will be accepted.
