- Applicants name must match (However shortened versions such as 'Chris' for Christopher can be accepted)
Certificate must have listed: - Use of Portable Firefighting Equipment OR
- First Attack Fire Fighting Training
Completion date or issue date must be displayed Name and / or signature of issuer
| - Issue date to be recorded as the date of training completion
- Expiry is 12 months from issue date

Certificate must have listed: - Fire Warden Training and Use of Portable Firefighting Equipment
OR Completion date or issue date must be displayed Name and / or signature of issuer
| - Issue date to be recorded as the date of training completion
- Expiry is 12 months from issue date

| Fire Fighting.Certificate.Demonstrate first attack firefighting equipment (CPPFES2005A) | - The applicants name shown on this certificate needs to match the applicants registered name in Onsite
- Demonstrate first attack firefighting equipment (CPPFES2005A) to be listed on certificate
- Certificate is to be issued by a Registered Training Organisation (RTO)
- Either black and white or colour copies of certification is accepted
- No expiry date is required to be recorded for this competency
| - The Issue date in Onsite should be recorded as the date of completion shown on the certificate
