• The form must be dated and must be signed by the WHC contract owner / coordinator
• Must be signed by relevant WHC manager if trade is being used
  Example: Manager Mechanical Engineering or Manager Electrical Engineering
• If going underground: Must be signed by manager mining engineering

Saved in the general folder (not the competency tab)

Whitehaven Open Cut:

• Gets uploaded to the generic comp: Safety.Induction.Open Cut and Generic ATUI Form

Whitehaven Underground:

• Admin. Letter of competency induction authorisation
  
  

• The form should be fully completed to be accepted

• The role of the person is listed i.e. Operator / Coal loader. Top of page 2.
• The primary Location must be listed.
• The induction would ideally be listed on the form however if this has been missed in completing the form it can  still be accepted
• For sign off:
  The contract owner or Contract coordinator always needs to sign.
  The mechanical or Electrical is only required if the role is linked to any mechanical or electrical role.
  The Manager of Mining engineering needs to sign for ALL underground trades and Operator for Narrabri.
  The Manager of Mining engineering needs to sign only for open cut operators.
