Licence.Work Activity.Electrical Refrigeration Air Conditioning

If a Sole Trader the name on Licence to match the person registered

If a Partnership the name on the Licence to match the Partnership registered

If a Company, the name on Licence to match the company registered

  • Air-conditioning, Refrigeration & Electrical to be listed (Ark Tick also acceptable)
  • Must be a Contractor Licence
  • Must be issued by State Government Authority
  • Duration as per licence
  • Black and white or colour is acceptable

Issue Date = As per licence (if not listed please leave blank)

Expiry Date = As per licence

 
Licence.Work Activity.Arc Tick

If a Sole Trader the name on Licence to match the person registered

If a Partnership the name on the Licence to match the Partnership registered

If a Company, the name on Licence to match the company registered

  • Must be an Arc Tick Licence Certificate of Card
  • Must be issued by State Government Authority
  • Duration as per licence
  • Black and white or colour is acceptable

Issue Date = As per licence (if not listed please leave blank)

Expiry Date = As per licence
