• First and last name must match applicant's name in Onsite
  • Shortened versions of first names are accepted, such as:
    "Chris" instead of "Christopher"
    "Wil" instead of "William"
  • Shortened versions of last names are accepted, such as:
    "Herrera" instead of "Herrera Trujillo"
    "Mohd" instead of "Raza Hasan Mohd"
• The uploaded evidence must be either of the following:
  
  • RTAY Recognition of Prior Competency
    1. Applicant's name
    2. Matching competency name and code
    3. Course completion date and/or expiry date
  • Theory Assessment or Site Authorisation
    1. All assessment pages within upload
    2. Matching competency name and code (if with checkbox, this should be completed)
    3. Current licence number and expiry date
    4. Applicant's signature and signed date
    5. If Site Authorisation, the RCC Assessment or CTO Assessment section must be completed
    6. Assessor's complete name, signature, and date
    7. Must show that the person is competent and authorised to operate
    8. If Theory Assessment, when RCC is completed, the authorised leader is required to verify with a signature that the applicant is authorised to operate.

NOTE: The learning and development section of the form does not need to be completed.

  • Progress Report
    1. Applicant's name

Issue date = If a certificate has been provided, use the course completion date as issue date. For Theory Assessment or Site Authorisation, use the assessor's signed date.

Expiry date = 2 years from issue date
