Additional Information Form

  • Please make sure that the additional information at the bottom is completed to say if assistance is required
  • All documents supplied MUST be one document per page (multiple competencies are not to be uploaded on the 1 page)
  • Will be saved in the general tab
 
Statement of Competency
  • Must include inductee details that match the person in the system
  • This can be the Pegasus Statement of Competency or on a company letter head
  • Must state what the person will be doing at site
  • Needs to be signed and verified by someone that oversees the work that the person will be doing.
  • All documents supplied MUST be one document per page; multiple competencies are not to be uploaded on the 1 page
  • The SOC competency needs to be entered ‘up to a date’ and needs to be 2 years from the induction date
