NOTE: Gunnedah CHPP does not have a MME – therefore this signature is not required for applicants that have indicated Gunnedah CHPP is as their primary WHC site even if the 'All Open Cut - Operator' role has been chosen. Page 2 - Inductions; Check list must be completed
