The ASP/1 Individual  Authorisation Agreement (older version of the form) OR the Level 1 Accredited Service Provider Individual Agreement for Authoirsation Form (newer version of the form) must be completed showing:

  • Applicant/ Individual First Name - name should be displayed on the evidence and should match the applicant name that's registered in Pegasus (However shortened versions of names such as 'Chris' for 'Christopher' is accepted.
  • Applicant/ Individual Surname
  • Applicant / Individual contact number
  • Applicant/ Individual Date of Birth (DOB) - DOB that is listed matches the applicants registered DOB in Pegasus.
  • Company Name - name on the form needs to match the company name that the individual is registered under within Pegasus

If the company name shown on the document is match on a trading or business name for the same company shown on the ABR website this is acceptable

We can accept ‘PTY LTD’ or ‘(Pty) Ltd’ in lieu of ‘Proprietary Limited’ when this is the only difference in the company name between what’s shown on company compliance paperwork verse what’s shown in Pegasus.

We can also accept the company name without ‘PTY LTD’ or ‘(Pty) Ltd or 'Limited / LTD' if this has been missed off the end of the company name when it was written on the form

  • Company AUC No
  • Authorization category (minimum of 1 to be selected) - This must match the category thats shown in the selected role for this applicant in Pegasus

Temporary process while COVID-19 restrictions are in force: Acceptance of Digital Signatures

Acceptance of digital signatures is based on the following criteria:

- Digitally signing with a stylus on a tablet; or
- Using a PDF program to insert the signature electronically

In the options above, the documents must be 'witnessed' by being digitally counter-signed by a third party in the same manner.

The use of cursive or script style fonts are not an acceptable digital PDF signature.

Documents submitted with digital signatures must be in PDF file format. Other documents such as Microsoft Word or Excel will be rejected.
