Covid Vaccination (can be mandated for specific sites)
  • Applicants name to be displayed on uploaded evidence and to match applicants registered name (however shortened versions of names such as 'Chris' for 'Christopher' is accepted)
  • Evidence must state completion of Covid Vaccination
  • Must be issued by Relevant Government Authority
  • Vaccination history accepted providing Covid-19 is listed
  • Black and white or colour is acceptable
  • Issue date to be recorded
