Welder.Statement of Attainment.Polyethylene Butt & Electro Fusion Welding
  • Name on certificate or card to match person registered (however shortened versions such as 'Chris' for 'Christopher' are acceptable)
  • Accepted Documents:
    • Statement of Attainment
    • Certificate of Attendance
    • Welders Card
  • Competency unit/qualification code must match exactly
  • Must be Issued by an RTO with Nationally Recognised Training logo
  • Black and white or colour is acceptable
  • Scanned document to be clearly legible


Note: If a card is supplied there must be a copy of the front and back displaying the units of competency, person's name, issue date as above and must be issued by an RTO.

  • Issue Date to be entered as date of training completion
  • Expiry date not required

Welder.Certificate.PMBWELD301 Butt Weld Polyethylene Plastic Pipelines

    • Welders Card
  • Competency unit/qualification code must match exactly
  • Must be Issued by an RTO with Nationally Recognised Training logo
  • Black and white or colour is acceptable
  • Scanned document to be clearly legible

Welder.Certificate.Certification Welding AS-NZS 1554-1 Certificate (or equivalent)
  • The worker’s name shown on the document should match the worker’s registered name in Onsite. First name initial and surname is acceptable
  • A weld test report must be submitted
  • The document will need to mention either Certificate Welding AS/NZS 1554.1 or AS/NZS 2980 or ISO 9606
  • Must have a C for competent
Record date of test as the issue date
