Licence.-.ADAS Licence - Diver	Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable) Evidence will be ADAS Licence showing 'Diver' as the achieved qualification Must be Issued by Australian Diver Accreditation Both sides of the licence must be displayed within uploaded file Black and white or colour is acceptable Scanned document to be clearly legible	Issue Date: Not required Expiry Date: As shown on the licence
