Drug and Alcohol Testing.-.Drug and Alcohol Testing (within 14 days of submission

      • Must have been completed within 14 days prior to the individuals submission date
      • Name on Drug and Alcohol Screening to match the registered person
      • Must be completed by an accredited testing facility by ensuring the document was issued by medical practice or pathology
      • D & A screening in accordance with AS4308 (urine standard) not AS4760 as this is an oral standard Urine testing shall screen for:
        -  Amphetamines
        - Benzodiazepines
        - Opiate
        - Cannabinoids metabolites
        - Cocaine Metabolites
      • Negative results should be shown to be acceptable**
      • Alcohol result must = 0.000% OR Negative

      If the Alcohol test result is greater than 0.000% then the application will be returned to the company for re-submission

  • Issue Date = Date of issue / test shown on results
    Expiry Date = no expiry to be entered

    **In the case that a positive drug and alcohol test result is supplied, laboratory confirmation that the drug which has been detected is consistent with declared medication must be submit with the drug and alcohol test result paperwork - All paperwork must then be supported by a declaration by the company too (all documentation should be included within the one upload); the declaration must:

    • Be presented on a company letterhead
    • Be signed by a senior company representative, e.g. supervisor, HR representative, return to work coordinator
    • Be dated within the last 3 months
    • State that the employee’s drug and alcohol screening paperwork has been approved by the company
