• Must have MAC Manager and employee signatures. NOTE: Sometimes Electronic signatures may be used, which is acceptable.
• Appointment Letter only required to be done annually if electrical and has been set to a one-year expiry date
• BHP MAC Electrical manages renewal of Appointment Form. 
• The role selected in onsite must match the role listed in the “Select Competency Area” field on the appointment letter. If the role does not match the letter cannot be accepted and must be returned to amend role selection.   

• Issue Date and Expiry Date to be recorded
• Issue Date = Date of Letter
• Expiry Date = 12 months from Issue Date

• Mechanical Roles require an Appointment form where “Selected Discipline” is Mechanical.

• The Role selected in Onsite must match the role listed in the “Select Competency Area” field or Match the Red Box Role approved by Stat Manager.
• Appointment form must have MAC Manager and Employee Signatures and Dated by both parties – Electronic Signatures are acceptable.
• The date of the appointment must be no older than 3 months.

• Issue Date to be recorded
• Issue Date = Date on Letter of Appointment
• Expiry Date =  5 years from Issue Date

• ABN must match the ABN of the company the employee is listed against in Onsite
• The company name must match the company the employee is listed against in Onsite
• Must have MAC Manager and employee signatures. NOTE: Sometimes Electronic signatures may be used, which is acceptable.
• The role selected in onsite must match the role listed in the “Select Competency Area” field on the appointment letter. If the role does not match the letter cannot be accepted and must be returned to amend role selection.
