Supervisor Training Certificate or Evidence of Supervisory Experience

• Contractor Company shall provide evidence of Supervisor training/qualification or evidence of experience in the role as a Supervisor.

• Applicant name to be displayed on certificate or letter.
• Applicant name on certificate or letter to match the person registered
• Supervisor Training Certificate or Letter of Supervisory Experience to be uploaded
• Date of completion must be shown on any certificate

• Example of Supervisor Training Certificate that is acceptable include: - must state

   -PWCS Work Training Course, or

  - Any other Supervisor/Frontline Leadership Training Course delivered and issued by a registered RTO.

• Letter of Supervisory Experience issued by Contractor Company must be issued on the Company Letterhead

  • Incident the number of years of supervisory experience
  • Be signed off by the applicants direct line manager/supervisor or more senior leader

• Completed date must be entered
