Generic.Apprentice Indenture Papers (3rd year minimum)Applicants name to be shown on document and must match cardholders registered name in Onsite (however shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)
Apprentice Indenture papers to be uploaded.
Must be issued by Department of Education in relevant state
Issue date to be recorded
Expiry date 4 years from issue date

Generic.Letter of Competency.Letter of Competency from Employer

The letter of competency provided by the employer of a contractor must include the following:

The name of the person deemed competent,
The name of the employing company,
The basis on which the person is deemed competent,
The time employed by the company,
The time the person deemed competent has performed the skill,
The name, position and signature of the person writing the letter on behalf of company (Company Directors/Owners may sign their own letters of competency)

Location of Work Area – Maxwell Infrastructure or Maxwell Underground
The time of mine site experience

Date on letter must be no older than 3 months prior to the date of data application submission
Issue date to be recorded in Onsite

Site.Authorisation.Maxwell Underground Approval to Work
  • Must be completed on the Maxwell Underground Approval to Work form with all sections completed 
  • Applicants name shown on document must match the applicants registered name in Onsite 
  • Approval to Work must only be uploaded by a Malabar User 
  • Approval to Work must be signed and dated
  • Black and white or colour copies are accepted
  • Must be clear and legible
