  • Must be a completed and approved version of the McArthur River Portal Role Approval Form (link to the form TBA)
  • Must show McArthur River Mine representative's approval:
    • Authoriser and Date sections of the form must be completed (if Authoriser is listed as 'other', this can be accepted)
  • Issue Date = Approval date (not submission date)
  • Expiry Date = 3 years from Issue Date
