Generic.-.Apprentice / Trainee Papers
  • Applicant name on paperwork to match the name of the registered applicant

  • Document must be Issued by a Registered Training Organisation (RTO) OR  Group Training Organisation (GTO) - Note: If the document provided is an Apprenticeship/Traineeship Training Contract, this document is accepted as long as the RTO details are listed.

  • Black and white or colour is acceptable

Note: Training Plans can only be provided if all pages are uploaded.

  • Issue date = date of commencement as listed on agreement
  • Expiry date = completion date as listed on agreement


If Apprenticeship/Traineeship Training Contract is uploaded:

  • Issue Date = Dated listed against 'Commencement date of Apprenticeship/Traineeship'
  • Expiry Date = Issue Date + 'Nominal term of Training Contract'

Generic.-.Apprenticeship Indenture Papers
  • Applicant name on paperwork to match the name of the registered applicant
  • Document must be Issued by a Registered Training Organisation (RTO) OR  Group Training Organisation (GTO)
  • Black and white or colour is acceptable
  •  There is no requirement to record and Issue date or an expiration date for this competency in Onsite

Generic.-.Traineeship Agreement
  • Applicant name on paperwork to match the name of the registered applicant
  • Document must be Issued by a Registered Training Organisation (RTO) OR  Group Training Organisation (GTO) - Note: If the document provided is an Apprenticeship/Traineeship Training Contract, this document is accepted as long as the RTO details are listed.
  • Black and white or colour is acceptable
    • Issue date = date of commencement as listed on agreement
    • Expiry date = completion date as listed on agreement

  • Issue Date = Dated listed against 'Commencement date of Apprenticeship/Traineeship'

 Generic.High Risk Work/Licence Competency Declaration

The Visy High Risk Work/Licence Competency Declaration form can be downloaded by clicking HERE

  • Applicant name on  High Risk Work/Licence Competency Declaration evidence to match the name of the registered applicant
  • High Risk Work/Licence(s) on form to match Competency in Onsite
  • Visy High Risk Work/Licence Competency Declaration Template to be used
  • All fields to be complete
  • Direct Managers signature* to be present (cannot be the contractor)

Note: Company Directors are the only individuals that can sign off their own Statement of Competency

  • Record the date signed as issue/completion date within Onsite
  • Expiry date - is always 2 years from the issue/completion date

The Visy statement of competency form can be downloaded by clicking HERE

  • Applicant name on Statement of Competency evidence to match the name of the registered applicant
  • Pegasus Visy Statement of Competency Template to be used
  • All fields to be complete however if the worker has been completing the activity for 5 years or more then the Previous Work History Section is not required to be filled in.
  • Direct Managers signature* to be present (cannot be the contractor)
