Builder.Licence.

If a Sole Trader the name on Licence to match the person registered
If a Company, the name on Licence to match the company registered
Building to be listed
Must be issued by relevant government department based on state of issue
Duration as per licence
Black and white of colour is acceptable
Relevant government department based on state of issue must be present

Note: Otis will accept a screen shot showing that the licence is current for an interim of 4 weeks and then the licence needs to be supplied
