  • Applicants name to be shown on document
  • Applicants name on document must match cardholders registered name (However shortened versions of first names such as 'Chris' for 'Christopher' is acceptable)
  • Uploaded document must be a logbook or work experience record
  • Verifier name and signature must be present on the document
  • Must be current
  • Issue Date = Date of upload
  • Expiry Date = Record as 1 year from issue date
