Operator.Certificate.TR-0004A – Risk Management in Service Stream Services - for managers

  • RTO Name, National Provider Code / RTO Number & Logo to be displayed on document
  • Name on certificate to match the name in the system (shortened versions accepted eg. ‘Chris’ accepted for Christopher)
  • Completion date or issue date must be displayed
  • Course title and associated level to be listed on certificate to match the nominated coding and title within Onsite
  • Document / Certificate / Number
  • Nationally Recognised Training logo
  • No expiry
  • Record the Issue date within Onsite:

Completion date - if listed on certificate (this completion date is always used when displayed)

Management.Certificate IV.BSB40807 - Frontline Management
  • Name on certificate to match the person registered
  • Certificate IV in Frontline Management (BSB40807) to be listed
  • Issued by an RTO / Nationally Recognised Training logo
  • Black and white or colour acceptable
  • Must be clearly scanned and legible
  • No Expiry
  • Issue date to be recorded

Leadership.Course.Service Stream Frontline Leaders Program

  • The applicants name shown on this statement of completion needs to match the applicants registered name in Onsite
  • Frontline Leader’s Program to be listed as complete on statement of completion
  • Either black and white or colour copies of certification is accepted

  • No expiry date is required to be recorded for this competency


  • The issue date in Onsite should be recorded as the date of completion shown on the statement of completion
Engineering.Authorisation Letter.Subject Matter Expert - Letter of Endorsement
  • Name on the SME Letter of Endorsement to match the person registered (Shortened version of name is accepted EG. Chris for Christopher)
  • All sections of the summary table must be completed (Full Name, Current Position, Tenure within Service Stream, Years of Experience, Highest Qualification, Engineering Disciplines, Chartered Status).
  • Approval à All signatures must be completed to recognise that the National Technical Training Manager and General Manager – Rail has endorsed and approved the SME.  SME must also countersign the document, acknowledging their role as a SME.
  • Issue date to be recorded in Onsite as reflected on the SME Letter of Endorsement.
  • EXPIRY: There is a 3 year expiry for this document.


 

Rules for renewal of SME competency:


In accordance with Engineering Competency Management Procedure, all employees who undertake an assessment must be reassessed every three [3] years. This same rule applies to Subject Matter Experts. In order to ensure each SME is continuing their own professional development – for example, maintaining chartered status, continuing CPD requirements, undertaking further training, keeping up-to-date with their engineering discipline / practice – SME’s will be reviewed and reassessed every three years also.


Expectations Upon Reassessment | Accepted Evidence:


The Rail General Manager can sign and date the original SME Letter of Endorsement to verify that the justification / reassessment has taken place. Accountability will be with the Rail GM to make this determination. The National Technical Training Team will re-upload this letter to be verified, as per the Rail GM’s assessment. Additional evidence (*such as further training & development, certificates, qualifications) gained throughout that three year timeframe will also be uploaded against the SME’s profile.

Operator Excavator Authorised Excavation Permit Authoriser (5 Co-Signed Permits)  (MUST be supervised by permit trained LLS supervisor)

  • Uploaded evidence will be an Enersafe issued certificate for permit authoriser
  • 5x records of the completed Excavation Permit form that shows the Service Stream Logo that shows experience in  Excavation across telco sites
  • Issue date = date of training completion
  • Expiry date = no expiry date
