• Name on certificate to match the person registered
• Certificate must be relevant to the role held (ie Electrical, Fitter etc)
• State issued Trade cards / Contractor licences that show relevant trade is accepted 
• Black and white or colour acceptable
• Must be clearly scanned and legible

• Issue Date = Date of completion
• Expiry Date = None
