Air Conditioning & Refrigeration (ACT)	If Sole Trader: The name on the Licence to match the Sole Trader name registered in the system If Partnership: The name on the Licence to match the Partnership name registered in the system If Company: The name on Licence to match the Company name registered in the system Must be a copy of the licence – Screenshots from websites are not accepted Must be issued by relevant state government appointed authority and display the corresponding logo Arc Tick licence can be accepted - must show relevant government authority or Arc Tick logo Duration to be recorded indicated on licence Black and white or colour copy of the document is acceptable	Issue Date = Date of completion / Issue if displayed on Licence Expiry Date = Date of expiry as displayed on Licence
