Certificate as an Electrical Supervisor or Contractors Licence	Name on qualification to match the person registered Accepted Evidence Types: Australian Government issued Electrcian Supervisor licence Australian Government issued Electrcian Contractor licence showing 'Q' in the bottom righthand corner of the licence Must be clearly scanned and legible Scanned in colour Front & Back of the Licence must be uploaded PDF copies only A digital copy of the Licence can be accepted	Expiry Date = Expiry date to be entered
