Mines rescue attendance sheet also accepted. The sheet must include the following; - Must be on Mines rescue template
- Must be current within 6 months
- Must state in the course name must include self-rescuer/self-escape training
- Must state the workers name, company, DOB and signature to ensure accurate.

- Expiry Date = 6 months from date of issue
