Welder.Statement of Attainment.Polyethylene Butt & Electro Fusion Welding	Name on certificate or card to match person registered (however shortened versions such as 'Chris' for 'Christopher' are acceptable) Accepted Documents: Statement of Attainment Certificate of Attendance Welders Card Competency unit/qualification code must match exactly Must be Issued by an RTO with Nationally Recognised Training logo Black and white or colour is acceptable Scanned document to be clearly legible Note: If a card is supplied there must be a copy of the front and back displaying the units of competency, person's name, issue date as above and must be issued by an RTO.	Issue Date to be entered as date of training completion Expiry date not required
